Royal jelly is generally well tolerated in most people. The main safety concern is allergic reaction — and it's a serious one if you have a history of asthma or atopy (a tendency toward allergic conditions).

In sensitive people, royal jelly can cause itching, hives, eczema, swelling of the eyelids and face, shortness of breath, and oral allergy syndrome. In rare cases, it can trigger anaphylaxis or severe asthma (status asthmaticus), even death.

There is also one case report of bleeding in the colon with severe abdominal pain and bloody diarrhea. There isn't enough safety data to use royal jelly during pregnancy or while breastfeeding.
