SAMe is likely effective for osteoarthritis. For depression, the evidence shows it's possibly effective.
It's also possibly effective for cholestasis (a liver condition involving impaired bile flow). For Alzheimer's disease and ADHD, the evidence we hold is insufficient to rate how well it works.
How safe is it?
Well-documented data
SAMe is generally well tolerated in typical doses, though side effects become more common at higher amounts. The most frequent ones are stomach upset — nausea, constipation, diarrhea, heartburn — along with headache, dizziness, insomnia, and nervousness.
A few people report rash, itching, or transient hair loss. One important caution: SAMe can trigger mania in people with bipolar disorder, so if that applies to you, talk with your doctor first.
Overall safety isn't well established, so use it only under a healthcare provider's guidance.

SAMe (S-adenosyl-L-methionine) is a molecule that is naturally formed in the body. SAMe is a methyl donor in a process essential to normal cell health and function known as methylation. The methylation process can decline with age or as a result of chronic alcohol consumption, and it can be limited by genetic defects. SAMe contributes to the synthesis, activation, and metabolism of neurotransmitters, proteins, nucleic acids, phospholipids, hormones, and some drugs. It supports glutathione production, liver health, joint comfort, and a healthy mood.
Formulated to exclude: Wheat, gluten, corn, yeast, soy, animal and dairy products, fish, shellfish, peanuts, tree nuts, egg, ingredients derived from genetically modified organisms (GMOs), artificial colors, artificial sweeteners, and artificial preservatives.

Formula

S-Adenosyl-L-Methionine
Levodopa
SAMe might reduce the effectiveness of levodopa.
SAMe methylates levodopa, which might reduce its effectiveness for treating Parkinson disease.
Serotonergic Drugs
Taking SAMe with serotonergic drugs might increase the risk of serotonin syndrome and other serotonergic side effects.
SAMe has serotonergic effects. Theoretically, combining serotonergic drugs with SAMe might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. In one case report, SAMe 100 mg intramuscularly was given daily with clomipramine (Anafranil) 25 mg per day. When the clomipramine dose was increased to 75 mg per day the patient experienced serotonin syndrome about 48-72 hours later, requiring hospitalization.
