hydrocortisone, pyrilamine, chlorpheniramine falls under SEROTONERGIC DRUGS — the group Same interacts with.

Taking SAMe with serotonergic drugs might increase the risk of serotonin syndrome and other serotonergic side effects.
SAMe has serotonergic effects. Theoretically, combining serotonergic drugs with SAMe might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. In one case report, SAMe 100 mg intramuscularly was given daily with clomipramine (Anafranil) 25 mg per day. When the clomipramine dose was increased to 75 mg per day the patient experienced serotonin syndrome about 48-72 hours later, requiring hospitalization.

This product is a topical liquid (HC Derma-Pax) that combines hydrocortisone with two antihistamines, pyrilamine and chlorpheniramine, used on the skin to calm itching and irritation. The concern on file links SAM-e with serotonin-type side effects, but honestly that worry is aimed at drugs that strongly raise serotonin in the brain. The ingredients in this skin product don't really work that way, and because you apply it to the skin, very little gets into your bloodstream.

So for this specific product, a meaningful interaction with SAM-e is unlikely, and the evidence behind the warning is weak (a single case report with a different, much stronger medicine). Still, if you ever take oral chlorpheniramine too, watch for extra drowsiness. When in doubt, check with your pharmacist.
