Same and Dextroamphetamine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

SEROTONERGIC DRUGS

Why this applies to Dextroamphetamine

Dextroamphetamine falls under SEROTONERGIC DRUGS — the group Same interacts with.

Taking SAMe with serotonergic drugs might increase the risk of serotonin syndrome and other serotonergic side effects.
SAMe has serotonergic effects. Theoretically, combining serotonergic drugs with SAMe might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. In one case report, SAMe 100 mg intramuscularly was given daily with clomipramine (Anafranil) 25 mg per day. When the clomipramine dose was increased to 75 mg per day the patient experienced serotonin syndrome about 48-72 hours later, requiring hospitalization.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: SAM-e seems to nudge serotonin activity upward in the brain. Dextroamphetamine (Dexedrine, Xelstrym) isn't a classic serotonin drug, but it does boost certain brain chemicals, and it can raise serotonin signaling a bit too. Stack the two together and you may, in theory, get more serotonin activity than you bargained for.

What could that look like? Feeling jittery, a racing or pounding heart, sweating, shaky muscles, agitation, or trouble sleeping, and in rare cases something more serious called serotonin syndrome. I want to be honest though: this concern is mostly theoretical and based on a single case report involving a different medication, so the real risk with dextroamphetamine is likely low. Still, please check with your pharmacist or doctor before combining them, and don't change your stimulant dose on your own.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Dextroamphetamine page

“Dextroamphetamine (Dexedrine) is a stimulant for ADHD and narcolepsy, and I read this list with genuine wariness. Six of the 129 ingredients rate major, most of them because other stimulants, ephedra being the clearest, pile extra heart rate and blood pressure on top of what the drug already produces.”
