Same and Eletriptan

Here is the interaction we found between these two, with the evidence behind it.

Moderate

SEROTONERGIC DRUGS

Why this applies to Eletriptan

Eletriptan falls under SEROTONERGIC DRUGS — the group Same interacts with.

Taking SAMe with serotonergic drugs might increase the risk of serotonin syndrome and other serotonergic side effects.
SAMe has serotonergic effects. Theoretically, combining serotonergic drugs with SAMe might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. In one case report, SAMe 100 mg intramuscularly was given daily with clomipramine (Anafranil) 25 mg per day. When the clomipramine dose was increased to 75 mg per day the patient experienced serotonin syndrome about 48-72 hours later, requiring hospitalization.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to know about pairing SAM-e with eletriptan (Relpax). Both nudge serotonin activity in your body. Eletriptan is a triptan that works on serotonin receptors to calm a migraine, and SAM-e can have serotonin-like effects of its own. Stacking them could, in theory, push serotonin too high and lead to serotonin syndrome, which can look like agitation, a racing heart, sweating, shivering, muscle twitching, or confusion.

I want to be honest about the evidence though. This concern comes mostly from one case report involving a different medication, so it's more of a theoretical caution than a proven problem. That said, if you take both, it's worth a quick chat with your pharmacist or doctor first, and seek help right away if you feel those symptoms.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.

From our pharmacist’s overview on the Eletriptan page

“Eletriptan (Relpax) treats migraines by acting on serotonin receptors, which is exactly why I want you to slow down with this list. Twenty of the 38 ingredients here also raise serotonin, and stacking them on a triptan risks serotonin syndrome, where too much serotonin brings agitation, sweating, and a racing heart.”
