SEROTONERGIC DRUGS
Why this applies to Benzocaine, Dextromethorphan
Benzocaine, Dextromethorphan falls under SEROTONERGIC DRUGS — the group Same interacts with.
SAMe has serotonergic effects. Theoretically, combining serotonergic drugs with SAMe might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. In one case report, SAMe 100 mg intramuscularly was given daily with clomipramine (Anafranil) 25 mg per day. When the clomipramine dose was increased to 75 mg per day the patient experienced serotonin syndrome about 48-72 hours later, requiring hospitalization.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's what matters in your Tetra-Formula combo: the dextromethorphan (the cough suppressant) gently nudges up serotonin activity in the brain. SAM-e does the same thing. Stack the two and, in theory, you could push serotonin a little too high. The benzocaine part is just a numbing agent and doesn't really factor in here.
To be honest, this concern is mostly theoretical and based on scattered case reports, not strong studies. Most people taking an occasional cough product won't notice anything. But if you take SAM-e daily, it's worth watching for signs like agitation, shakiness, sweating, a racing heart, or feeling confused. If that happens, stop and get help. Before mixing them regularly, please check with your pharmacist or doctor.

From our pharmacist’s overview on the Benzocaine, Dextromethorphan page
“Tetra-Formula pairs benzocaine, a throat-numbing agent, with dextromethorphan, a cough suppressant that raises serotonin activity, and that second ingredient explains why 21 of the 24 supplement interactions on file share one theme, pushing serotonin higher and raising the risk of serotonin syndrome (agitation, racing heart, fever, con…”
