Palonosetron falls under SEROTONERGIC DRUGS — the group Same interacts with.

Taking SAMe with serotonergic drugs might increase the risk of serotonin syndrome and other serotonergic side effects.
SAMe has serotonergic effects. Theoretically, combining serotonergic drugs with SAMe might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. In one case report, SAMe 100 mg intramuscularly was given daily with clomipramine (Anafranil) 25 mg per day. When the clomipramine dose was increased to 75 mg per day the patient experienced serotonin syndrome about 48-72 hours later, requiring hospitalization.

Here's the deal with combining SAM-e and palonosetron (Aloxi). Palonosetron works on serotonin too, but in a different way: it actually blocks certain serotonin receptors to keep you from getting sick to your stomach after chemo or surgery. SAM-e, on the other hand, can gently nudge serotonin activity upward. When you stack two things that touch the serotonin system, there's a small, mostly theoretical worry about something called serotonin syndrome, which can cause agitation, a racing heart, shivering, sweating, or muscle twitching.

Honestly, the evidence here is thin (just a few anecdotal reports), and palonosetron is usually a short, one-time dose given by your cancer team. Still, please mention you take SAM-e to your doctor or pharmacist so they can keep an eye on you.

What Is Palonosetron Injection Used For?

Brand name: Aloxi

Palonosetron hydrochloride injection is used in adults to prevent nausea and vomiting caused by moderately or highly emetogenic (nausea-causing) cancer chemotherapy — including both the immediate reaction and the delayed reaction that can happen days later. It is also approved in adults to prevent postoperative nausea and vomiting (PONV) for up to 24 hours after surgery, given right before anesthesia is started.

In children aged 1 month to less than 17 years, it is used to prevent acute (immediate) nausea and vomiting from emetogenic cancer chemotherapy, including highly emetogenic regimens. Posfrea is the brand name under which this intravenous injection is available.
