SEROTONERGIC DRUGS
Why this applies to Duloxetine
Duloxetine falls under SEROTONERGIC DRUGS — the group Same interacts with.
SAMe has serotonergic effects. Theoretically, combining serotonergic drugs with SAMe might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. In one case report, SAMe 100 mg intramuscularly was given daily with clomipramine (Anafranil) 25 mg per day. When the clomipramine dose was increased to 75 mg per day the patient experienced serotonin syndrome about 48-72 hours later, requiring hospitalization.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version: duloxetine (Cymbalta) already raises serotonin levels in your brain, since that's how it eases depression, anxiety, and nerve pain. SAM-e seems to nudge serotonin upward too. Stacking the two could, in theory, push serotonin a bit too high and lead to what's called serotonin syndrome.
What might that look like? Things like agitation, a racing heart, sweating, shivering, muscle twitching, or feeling shaky and confused. I'll be straight with you, though: this concern is mostly theoretical and rests on a single case report with a different medicine, so it's far from guaranteed. Still, I wouldn't combine them on your own. Loop in your pharmacist or doctor first, and if you ever feel those symptoms coming on, get help promptly.

Duloxetine treats major depressive disorder and generalized anxiety disorder. It also treats pain conditions: diabetic peripheral neuropathic pain (nerve pain from diabetes), fibromyalgia, and chronic musculoskeletal pain (such as chronic low back pain and osteoarthritis pain).
Not every product is labeled for every age group. Cymbalta and some duloxetine products include generalized anxiety disorder in children 7 and older and fibromyalgia in teens 13 and older. Drizalma Sprinkle is labeled for fibromyalgia in adults. Some other duloxetine products do not carry the added pediatric information.
