Same and Phenylephrine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

SEROTONERGIC DRUGS

Why this applies to Phenylephrine

Phenylephrine falls under SEROTONERGIC DRUGS — the group Same interacts with.

Taking SAMe with serotonergic drugs might increase the risk of serotonin syndrome and other serotonergic side effects.
SAMe has serotonergic effects. Theoretically, combining serotonergic drugs with SAMe might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. In one case report, SAMe 100 mg intramuscularly was given daily with clomipramine (Anafranil) 25 mg per day. When the clomipramine dose was increased to 75 mg per day the patient experienced serotonin syndrome about 48-72 hours later, requiring hospitalization.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the honest picture with SAM-e and phenylephrine. SAM-e can nudge up serotonin activity in the body, and phenylephrine is grouped with drugs that have some serotonergic potential. In theory, combining them could raise the small risk of too much serotonin (called serotonin syndrome), with things like restlessness, a racing heart, sweating, shivering, or feeling jittery. Phenylephrine on its own can already bump up blood pressure and heart rate, so that overlap is the main concern.

I want to be straight with you though: this is based on weak, anecdotal evidence, and the real case involved a different, much stronger drug. With an occasional dose of a cold or sinus product, this is unlikely to be a problem for most people. Still, run it by your pharmacist or doctor first, especially if you have high blood pressure or take other mood medicines.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)

From our pharmacist’s overview on the Phenylephrine page

“If you take phenylephrine (Sudafed PE, Biorphen) for congestion, treat this list as one that needs real attention, since six of its 81 entries carry a major rating. Phenylephrine tightens blood vessels to clear your nose, and more than half of the interactions here come from stimulant ingredients, ephedra above all, that can push blood…”
