SEROTONERGIC DRUGS
Why this applies to Brexpiprazole
Brexpiprazole falls under SEROTONERGIC DRUGS — the group Same interacts with.
SAMe has serotonergic effects. Theoretically, combining serotonergic drugs with SAMe might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. In one case report, SAMe 100 mg intramuscularly was given daily with clomipramine (Anafranil) 25 mg per day. When the clomipramine dose was increased to 75 mg per day the patient experienced serotonin syndrome about 48-72 hours later, requiring hospitalization.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the deal with combining SAM-e and brexpiprazole (Rexulti). SAM-e can gently nudge up serotonin activity in the brain, and brexpiprazole also touches serotonin pathways. When you stack two things that both raise serotonin, there's a theoretical chance of too much serotonin building up, something we call serotonin syndrome. Signs to watch for include feeling restless or agitated, shivering, sweating, a fast heartbeat, muscle twitches, or confusion.
That said, I want to keep this in perspective. The evidence here is anecdotal, basically a case report, and it isn't a well-proven, common reaction with brexpiprazole specifically. It's worth a quick chat with your pharmacist or doctor before adding SAM-e, especially since brexpiprazole is often used alongside an antidepressant that also raises serotonin.

Brexpiprazole is used along with an antidepressant to treat major depressive disorder (MDD) in adults. It is also used to treat schizophrenia.
Product details differ. Rexulti is also approved for schizophrenia in teens 13 and older and for agitation associated with dementia due to Alzheimer's disease. The Brexpiprazole tablets label lists adult MDD (add-on) and adult schizophrenia.
