Fentanyl citrate falls under SEROTONERGIC DRUGS — the group Same interacts with.

Taking SAMe with serotonergic drugs might increase the risk of serotonin syndrome and other serotonergic side effects.
SAMe has serotonergic effects. Theoretically, combining serotonergic drugs with SAMe might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. In one case report, SAMe 100 mg intramuscularly was given daily with clomipramine (Anafranil) 25 mg per day. When the clomipramine dose was increased to 75 mg per day the patient experienced serotonin syndrome about 48-72 hours later, requiring hospitalization.

Here's the gist for fentanyl: this is a powerful opioid pain medicine, but it also has some serotonin activity in the brain. SAM-e can raise serotonin levels too, so taking the two together could, in theory, add up and increase the chance of a rare but serious reaction called serotonin syndrome. Warning signs include agitation, a racing heart, shivering, sweating, muscle twitching, or confusion. SAM-e doesn't make fentanyl build up or wear off faster, it's the overlapping serotonin effect that matters here.

To be honest, the evidence is mostly theoretical and based on a single case report with a different drug, so this isn't a strong red flag. Still, given how potent fentanyl is, please check with your doctor or pharmacist before pairing them.
