Benzocaine, Dextromethorphan falls under SEROTONERGIC DRUGS — the group Black Seed interacts with.

Theoretically, combining serotonergic drugs with black seed might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders.
Animal research suggests that black seed can increase brain serotonin levels. In one case report, a 35-year-old man undergoing endoscopic surgery experienced immediate postoperative serotonin syndrome that was likely associated with the use of black seed oil 600 mg daily starting 4 days before surgery, and precipitated by the use of serotonergic pain medications, including fentanyl and oxycodone. Monitor patients for signs of serotonin syndrome and other serotonergic side effects if using black seed with serotonergic drugs.

The piece of your cough and sore-throat product I want to focus on is the dextromethorphan, the cough suppressant. Dextromethorphan nudges serotonin levels up in the brain, and some animal research suggests black seed (Nigella sativa) may do the same. Stack the two together and, in theory, you could push serotonin too high, raising the risk of what we call serotonin syndrome.

Honestly, this is a theoretical concern based mostly on animal data and a single surgical case report, so I don't want to alarm you. Still, it's worth knowing the warning signs: agitation, a racing heart, sweating, shivering, muscle twitching, or feeling confused. Concentrated black seed oil capsules are more of a question mark than a sprinkle in cooking. If you take both, check in with your pharmacist or doctor first.
