Maprotiline falls under SEROTONERGIC DRUGS — the group Same interacts with.

Taking SAMe with serotonergic drugs might increase the risk of serotonin syndrome and other serotonergic side effects.
SAMe has serotonergic effects. Theoretically, combining serotonergic drugs with SAMe might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. In one case report, SAMe 100 mg intramuscularly was given daily with clomipramine (Anafranil) 25 mg per day. When the clomipramine dose was increased to 75 mg per day the patient experienced serotonin syndrome about 48-72 hours later, requiring hospitalization.

Maprotiline is an antidepressant that raises serotonin and norepinephrine levels in your brain. SAM-e has its own mild serotonin-boosting effect. Put the two together and you stack two things pushing serotonin in the same direction, which can raise the small chance of serotonin syndrome. Watch for things like agitation, a racing heart, sweating, shivering, muscle twitching, or confusion. If those show up, get medical help right away.

I want to be honest about the evidence here: this concern is mostly theoretical and based on a single case report with a different antidepressant, so it is not common. Still, the combination is worth a conversation. Please check with your pharmacist or doctor before adding SAM-e, and don't stop or change your maprotiline on your own.

  • Maprotiline is used to treat depression, bipolar disorder (manic depressive disorder; a disease that causes episodes of depression, episodes of mania, and other abnormal moods), and anxiety. Maprotiline is in a class of medications called tetracyclic antidepressants. It works by increasing the amounts of certain natural substances in the brain that are needed to maintain mental balance.
