Acetaminophen, Chlorpheniramine, Phenylephrine, Salicylamide falls under SEROTONERGIC DRUGS — the group Same interacts with.
SAMe has serotonergic effects. Theoretically, combining serotonergic drugs with SAMe might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. In one case report, SAMe 100 mg intramuscularly was given daily with clomipramine (Anafranil) 25 mg per day. When the clomipramine dose was increased to 75 mg per day the patient experienced serotonin syndrome about 48-72 hours later, requiring hospitalization.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combo product is mainly a cold and allergy reliever: acetaminophen and salicylamide for aches, chlorpheniramine for sneezing and runny nose, and phenylephrine as a decongestant. None of these are strongly serotonin-boosting drugs on their own, so the concern here is pretty mild for most people. SAM-e can gently raise serotonin activity, and phenylephrine is a stimulant-type decongestant, so in theory the pair could add a little extra restlessness, jitteriness, fast heartbeat, or raised blood pressure.
Keep in mind this caution comes from weak, anecdotal evidence (a case involving a much stronger antidepressant), so true serotonin syndrome is unlikely from this product. Still, if you take SAM-e along with other serotonin-affecting medicines, run it by your pharmacist or doctor first.
