Chlorpheniramine Maleate, Dextromethorphan Hydrobromide falls under SEROTONERGIC DRUGS — the group Same interacts with.

Taking SAMe with serotonergic drugs might increase the risk of serotonin syndrome and other serotonergic side effects.
SAMe has serotonergic effects. Theoretically, combining serotonergic drugs with SAMe might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. In one case report, SAMe 100 mg intramuscularly was given daily with clomipramine (Anafranil) 25 mg per day. When the clomipramine dose was increased to 75 mg per day the patient experienced serotonin syndrome about 48-72 hours later, requiring hospitalization.

Here's the piece that matters with this cough and cold combo: the dextromethorphan part has serotonin-boosting activity in the brain. SAM-e nudges serotonin up too. Stack them together and, in theory, you could push serotonin a little too high, which is where serotonin syndrome comes from. Honestly, this is a pretty uncommon event, and the evidence here is mostly a single case report with a different drug, so I don't want to alarm you.

Still, it's worth knowing the warning signs: agitation, a racing heart, sweating, shivering, muscle twitching, or feeling confused. The chlorpheniramine side can also add drowsiness. If you take SAM-e regularly, run it by your pharmacist or doctor before reaching for this product, especially at higher or frequent doses.
