Acetaminophen, Chlorpheniramine, Dextromethorphan, Pseudoephedrine falls under SEROTONERGIC DRUGS — the group Same interacts with.

Taking SAMe with serotonergic drugs might increase the risk of serotonin syndrome and other serotonergic side effects.
SAMe has serotonergic effects. Theoretically, combining serotonergic drugs with SAMe might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. In one case report, SAMe 100 mg intramuscularly was given daily with clomipramine (Anafranil) 25 mg per day. When the clomipramine dose was increased to 75 mg per day the patient experienced serotonin syndrome about 48-72 hours later, requiring hospitalization.

This combination cold and cough product has one piece worth flagging: the dextromethorphan (the cough suppressant). Dextromethorphan can nudge serotonin activity in the brain, and so can SAM-e. Stacking two serotonin-raising things together can, in rare cases, push serotonin too high. That is called serotonin syndrome, and signs include agitation, a racing heart, sweating, shivering, muscle twitching, or feeling confused. The other parts (acetaminophen, chlorpheniramine, pseudoephedrine) are not the concern here.

To be fair, this risk is mostly theoretical and based on scattered case reports, not strong studies. Occasional short-term use for a cold is unlikely to cause trouble for most folks. Still, since you would be doubling up on serotonin effects, it is smart to run it by your pharmacist or doctor first.
