Netupitant, Palonosetron (Oral) falls under SEROTONERGIC DRUGS — the group Same interacts with.

Taking SAMe with serotonergic drugs might increase the risk of serotonin syndrome and other serotonergic side effects.
SAMe has serotonergic effects. Theoretically, combining serotonergic drugs with SAMe might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. In one case report, SAMe 100 mg intramuscularly was given daily with clomipramine (Anafranil) 25 mg per day. When the clomipramine dose was increased to 75 mg per day the patient experienced serotonin syndrome about 48-72 hours later, requiring hospitalization.

Here's the thing to know if you're using Akynzeo (netupitant and palonosetron) during chemo: the palonosetron part works on serotonin in your body. SAM-e also nudges serotonin activity. Stacking two things that raise serotonin can, in theory, increase the chance of too much serotonin building up, called serotonin syndrome. Watch for symptoms like agitation, a racing heart, shivering, muscle twitching, sweating, or feeling confused.

I want to be honest about the strength of this: it's mostly theoretical and based on scattered case reports, not strong studies, so for most people it's a low risk. Still, since you're managing chemo nausea, please run SAM-e by your oncology team or pharmacist before adding it.

What Is Netupitant and Palonosetron Used For?

Brand name: Akynzeo

Akynzeo capsules are used in adults to prevent both the short-term (acute) and longer-lasting (delayed) nausea and vomiting that can follow cancer chemotherapy — including, but not limited to, highly emetogenic (strongly nausea-causing) chemotherapy regimens. It is always taken together with the steroid dexamethasone.

Akynzeo capsules cover a broad range of chemotherapy regimens, including anthracycline plus cyclophosphamide-based regimens. Each ingredient targets a different stage: palonosetron addresses nausea and vomiting in the first 24 hours, while netupitant covers both the early and the delayed phase that can last several days after treatment.
