Fosnetupitant, Palonosetron (IV) falls under SEROTONERGIC DRUGS — the group Same interacts with.

Taking SAMe with serotonergic drugs might increase the risk of serotonin syndrome and other serotonergic side effects.
SAMe has serotonergic effects. Theoretically, combining serotonergic drugs with SAMe might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. In one case report, SAMe 100 mg intramuscularly was given daily with clomipramine (Anafranil) 25 mg per day. When the clomipramine dose was increased to 75 mg per day the patient experienced serotonin syndrome about 48-72 hours later, requiring hospitalization.

Here's the gist: one part of Akynzeo, the palonosetron, works by acting on serotonin in your body to calm nausea and vomiting. SAM-e also nudges serotonin activity up. When you stack two things that both raise serotonin, there's a small theoretical chance of too much serotonin at once, which doctors call serotonin syndrome. Signs can include agitation, a racing heart, sweating, shivering, muscle twitching, or feeling confused.

I want to keep this in perspective. This warning comes from anecdotal reports, not strong studies, and Akynzeo is usually given as a single dose around chemo. Still, since you're already going through chemotherapy, please mention any SAM-e use to your oncology team before your treatment so they can keep an eye out.

What Is Netupitant and Palonosetron Used For?

Brand name: Akynzeo

Akynzeo capsules are used in adults to prevent both the short-term (acute) and longer-lasting (delayed) nausea and vomiting that can follow cancer chemotherapy — including, but not limited to, highly emetogenic (strongly nausea-causing) chemotherapy regimens. It is always taken together with the steroid dexamethasone.

Akynzeo capsules cover a broad range of chemotherapy regimens, including anthracycline plus cyclophosphamide-based regimens. Each ingredient targets a different stage: palonosetron addresses nausea and vomiting in the first 24 hours, while netupitant covers both the early and the delayed phase that can last several days after treatment.
