Chlorpheniramine Tannate, Phenylephrine Tannate falls under SEROTONERGIC DRUGS — the group Same interacts with.

Taking SAMe with serotonergic drugs might increase the risk of serotonin syndrome and other serotonergic side effects.
SAMe has serotonergic effects. Theoretically, combining serotonergic drugs with SAMe might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. In one case report, SAMe 100 mg intramuscularly was given daily with clomipramine (Anafranil) 25 mg per day. When the clomipramine dose was increased to 75 mg per day the patient experienced serotonin syndrome about 48-72 hours later, requiring hospitalization.

Here's the thing about pairing SAM-e with Ricobid, which contains chlorpheniramine (an antihistamine) and phenylephrine (a decongestant). Chlorpheniramine has mild serotonin-related activity, and SAM-e can nudge serotonin levels up too. In theory, taking them together could add up and raise the small chance of serotonin syndrome, which can cause agitation, a racing heart, sweating, shakiness, or confusion.

I want to be honest with you: this concern rests on weak, anecdotal evidence (a single case report involved a much stronger antidepressant, not a cold medicine). So for most people taking Ricobid occasionally, the real-world risk is low. Still, since phenylephrine can already raise blood pressure and heart rate, it's worth a quick chat with your pharmacist or doctor before combining them, especially if you use SAM-e daily.
