The evidence for these ingredients is limited. Passionflower and valerian are both rated possibly effective for insomnia and pre-procedural anxiety (calming before medical procedures), and that's where the strongest data sits.
Bacopa, however, shows insufficient reliable evidence for the conditions it's traditionally used for — including memory problems, ADHD, and cognitive function — so we can't confirm it works for those purposes based on the research we have. If you're considering this product for a specific concern, it's worth discussing with your doctor what the evidence actually supports.
How safe is it?
Well-documented data
Bacopa and passionflower are generally well tolerated short-term, though bacopa's long-term safety isn't well studied. The most common side effects from bacopa are abdominal cramps, diarrhea, dry mouth, headache, and nausea; gastrointestinal upset occurs in 12 to 30 percent of people who take it.
Passionflower can cause drowsiness, dizziness, and confusion. Valerian is generally well tolerated but causes drowsiness and mental slowness; if you stop it abruptly after long-term use, you may experience withdrawal symptoms like anxiety, irritability, and insomnia, so taper slowly.
All three ingredients are not recommended in pregnancy or while breastfeeding due to insufficient safety data.

Before taking this product, check with your doctor or pharmacist if you use sedative medications (including sleep aids, anti-anxiety drugs like alprazolam, or pain relievers with sedative effects) — this product can add to their drowsiness. Avoid combining this product with alcohol, and let your pharmacist know if you take anticholinergic drugs or any medication metabolized by glucuronidation.

Serenity by Hawaii Pharm contains three herbal ingredients — bacopa, passionflower, and valerian — that together interact with a significant number of medications.
The most serious concern is a Moderate-severity interaction between bacopa and several drug-metabolizing enzymes (CYP1A2, CYP2C19, CYP2C9, and CYP3A4 substrates), which could theoretically raise the blood levels and side effects of medications processed through these pathways. Bacopa also interacts with anticholinergic and cholinergic drugs by opposing their effects, and with cevimeline (a medication for dry mouth in Sjögren syndrome).

Passionflower adds Moderate-severity interactions with sedative drugs (CNS depressants) through its own sedative effects, and Minor interactions with certain drug transporters and CYP3A4 substrates. Valerian contributes Moderate-severity interactions with sedatives and alcohol, and with drugs processed by glucuronidation; it also specifically interacts with alprazolam.
Together with bacopa's enzyme effects, valerian's Minor interactions with CYP2D6 and CYP3A4 substrates mean many common medications warrant review.

Brahmi Whole Herb Extract, Dry
Anticholinergic Drugs
Theoretically, concurrent use might decrease the effectiveness of both agents.
Bacopa seems to inhibit acetylcholinesterase and might increase acetylcholine levels, which could counteract the effects of anticholinergic drugs. Similarly, anticholinergic drugs might counteract the cholinergic effects of bacopa.
Cevimeline (Evoxac)
Theoretically, bacopa might increase the effects and adverse effects of cevimeline.
In one case, a 58-year-old female taking cevimeline long-term for Sjogren syndrome experienced hyperhidrosis, malaise, nausea, and tachycardia shortly after taking a single dose of bacopa. Symptoms resolved after two days. Cevimeline is metabolized by cytochrome P450 (CYP) 2D6 and CYP3A4, and researchers theorize that bacopa may have inhibited these isoenzymes. However, it is unclear if bacopa causes clinically significant inhibition of either CYP2D6 or CYP3A4.
Cholinergic Drugs
Theoretically, concurrent use of bacopa with other cholinergic drugs might have additive effects.
Bacopa seems to inhibit acetylcholinesterase and might increase acetylcholine levels. Theoretically, this could result in additive cholinergic effects when used with cholinergic drugs.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, bacopa might increase the levels and adverse effects of CYP1A2 substrates.
Research on the effects of bacopa extracts on CYP1A2 enzymes is conflicting. Some in vitro evidence shows that bacopa extract can moderately and non-competitively inhibit CYP1A2, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.
Cytochrome P450 2C19 (Cyp2C19) Substrates
Theoretically, bacopa might increase the levels and adverse effects of CYP2C19 substrates.
In vitro evidence suggests that bacopa extract can moderately and non-competitively inhibit CYP2C19 enzymes. It is not known whether this is clinically significant.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, bacopa might increase the levels and adverse effects of CYP2C9 substrates.
Research on the effect of bacopa extracts on CYP2C9 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and non-competitively inhibit CYP2C9, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, bacopa might increase the levels and adverse effects of CYP3A4 substrates.
Research on the effects of bacopa extracts on CYP3A4 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and competitively inhibit CYP3A4, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.
Thyroid Hormone
Theoretically, bacopa might have additive effects when used with thyroid hormone.
Animal research suggests that bacopa increases thyroxine (T4) levels in mice by about 40%.
Valerian Root Extract, Dry
Alcohol (Ethanol)
Valerian can have additive sedative effects when used concomitantly with alcohol.
Valerian has sedative effects. Theoretically, valerian might have an additive sedative effect when combined with alcohol. Excessive sedation has been reported in an alcohol-abusing individual who took valerian and Gingko biloba. However, the potential interaction between valerian and alcohol has been disputed in other research. Limited evidence suggests that a combination of valerian 160 mg and lemon balm 80 mg (Euvegal) does not cause further deterioration in reaction ability and reaction rate when taken with alcohol as compared to the effects of alcohol alone.
Alprazolam (Xanax)
Valerian can have additive sedative effects when used with alprazolam. Also, valerian in high doses might modestly increase alprazolam levels, though this is not likely to be clinically significant.
Valerian has sedative effects. Theoretically, valerian might cause additive sedation when combined with alprazolam. Also, a small pharmacokinetic study shows that taking valerian extract 1000 mg daily (providing 11 mg valerenic acid) might increase alprazolam levels by about 19%. This might be due to valerian's mild inhibition of cytochrome P450 3A4 (CYP3A4). Despite being statistically significant, this increase is not likely to be clinically significant.
Cns Depressants
Valerian can have additive sedative effects when used concomitantly with CNS depressant drugs.
Theoretically, concomitant use of valerian and drugs with sedative and anesthetic properties may cause additive therapeutic and adverse effects.
Glucuronidated Drugs
Valerian might weakly inhibit glucuronidation and increase concentrations of drugs metabolized by UGT1A1 and UGT2B7.
In vitro research shows that methanolic valerian extract and valerenic acid might competitively inhibit UDP-glucuronosyltransferase (UGT) 1A1 (UGT1A1) and UGT2B7.
Cytochrome P450 2D6 (Cyp2D6) Substrates
Valerian does not seem to have a clinically relevant effect on levels of drugs metabolized by CYP2D6.
Although some in vitro evidence suggests that valerian affects CYP2D6, clinical pharmacokinetic (PK) studies show that valerian is unlikely to affect the CYP2D6 enzyme. In one PK study, taking valerian 1000 mg (providing about 11 mg valerenic acid) nightly for 14 days did not affect the metabolism of dextromethorphan, a CYP2D6 substrate. In another PK study, taking valerian 125 mg three times daily for 28 days did not affect metabolism of debrisoquine, an accepted CYP2D6 probe-substrate.

Passionflower Herb Extract, Dry
Cns Depressants
Concomitant use of passion flower with sedative drugs might cause additive effects and side effects.
Research in animals and humans shows that passion flower has sedative effects which can be additive when used with sedative medications like lorazepam.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, passion flower might decrease the effects of CYP3A4 substrates.
In vitro research suggests that passion flower can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.
Organic Anion-Transporting Polypeptide Substrates (Oatp)
Theoretically, passion flower might reduce the bioavailability of OATP2B1 and OATP1A2 substrates.
In vitro research shows that the passion flower constituents apigenin and vitexin inhibit OATP2B1 and OATP1A2. This inhibition may be dose-dependent. One specific high-flavonoid passion flower extract (Valverde) seems to inhibit OATP2B1 and OATP1A2, while another extract with a lower flavonoid concentration (Arkocaps) shows less potent inhibition. OATPs are responsible for the uptake of drugs and other compounds into the body; however, the specific activities of OATP2B1 and OATP1A2 are not well characterized.
