Acetaminophen, Dextromethorphan, Phenylpropanolamine, Pyrilamine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Bacopa interacts with.
Research on the effects of bacopa extracts on CYP1A2 enzymes is conflicting. Some in vitro evidence shows that bacopa extract can moderately and non-competitively inhibit CYP1A2, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This cold-and-cough combo (Theracaps) blends acetaminophen, dextromethorphan, phenylpropanolamine, and pyrilamine. The concern here is that bacopa might gently slow down a liver enzyme called CYP1A2, which helps clear some of these ingredients. In theory, that could let the medicine build up a little, possibly nudging up side effects like drowsiness, dizziness, dry mouth, or feeling jittery from the decongestant.
Honestly, though, this is on shaky ground. The evidence comes only from lab dish studies, and even those disagree about whether bacopa does much at all. For most people taking a short course of a cold remedy, this is unlikely to cause real trouble. Still, if you take bacopa regularly, it is worth a quick word with your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Research on the effects of bacopa extracts on CYP3A4 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and competitively inhibit CYP3A4, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This Theracaps product is a combo cold remedy with four ingredients, and a couple of them (especially the cough suppressant dextromethorphan) get processed by a liver enzyme called CYP3A4. The thinking here is that bacopa might slow that enzyme down a little, which could let those ingredients build up more than usual. If that happened, you might notice extra drowsiness, dizziness, or feeling foggy.
That said, I want to be honest about how thin this evidence is. It comes only from lab tests, not real people, and even those studies disagree on whether bacopa does much at all. For a short course of a cold product, I wouldn't lose sleep over it. Still, it's worth a quick word with your pharmacist, especially if you take bacopa daily.

ANTICHOLINERGIC DRUGS
3 of 3Acetaminophen, Dextromethorphan, Phenylpropanolamine, Pyrilamine also falls under ANTICHOLINERGIC DRUGS.
Bacopa seems to inhibit acetylcholinesterase and might increase acetylcholine levels, which could counteract the effects of anticholinergic drugs. Similarly, anticholinergic drugs might counteract the cholinergic effects of bacopa.

This combination cold-and-allergy product (Theracaps) includes pyrilamine, an antihistamine that has anticholinergic (drying) effects. Bacopa appears to work in the opposite direction: in lab studies it may raise levels of acetylcholine, one of the brain's messenger chemicals. So in theory, the two could blunt each other a little. Bacopa might slightly weaken pyrilamine's drying action, and pyrilamine might slightly offset whatever memory-related effect you're hoping to get from bacopa.
I want to be honest about how soft this is. It's based on how these compounds behave in the lab, not on real-world reports of problems, so for most people it's unlikely to cause anything noticeable. If you take both regularly, just mention it to your pharmacist so they can keep an eye on things.

Bacopa is an Ayurvedic herb most often used for memory and thinking. Some small studies suggest it may modestly help memory when taken regularly for several weeks, but the evidence is limited and quality varies.
