Rehmannia and Poria mushroom are generally well tolerated in traditional use, but human safety data are limited and product quality varies. Paeonia (peony) is considered well tolerated for short-term use, though it can cause gastrointestinal side effects — abdominal distension, loss of appetite, diarrhea, nausea — in some patients.

One clinical study reported diarrhea in 5% of patients on a peony supplement at a higher dose versus 1% on placebo. Rare allergic reactions to poria, including allergic rhinitis and allergic asthma, have been reported.

For pregnancy and breastfeeding, the safety data aren't established enough. However, preliminary research in humans shows that peony root does not alter levels of losartan (Cozaar), which is also metabolized by CYP2C9.
