Stinging nettle is generally well tolerated by most adults when used as directed, though constipation and diarrhea have been reported. In rare cases, liver inflammation has been reported after use of herbal products containing stinging nettle, though those products also contained other ingredients.

If you touch or ingest fresh plant material directly, you may experience itching, rash, stinging, or in severe cases (like placing a leaf on your tongue), swelling and pain lasting several hours. Pregnancy safety data advises against use—there isn't enough data to know if it's safe, and traditional concerns about effects on the uterus mean you should avoid it and talk with your doctor.
