The evidence for shilajit is not established in our data — we hold insufficient reliable evidence for use in Alzheimer disease, sexual dysfunction, athletic performance, benign prostatic hyperplasia, or osteopenia. Barley is likely effective for high cholesterol and heart disease prevention based on clinical research.
Gokhshur (tribulus) is possibly effective for sexual dysfunction but appears possibly ineffective for athletic performance; evidence for benign prostatic hyperplasia is insufficient. For the remaining ingredients — punarnava, pashanbhed, shwet parpati, sahadev, palash pushap, varun ghana, apamarg, and lajjalu mool — we hold no effectiveness data.
How safe is it?
Well-documented data
Shilajit is generally well tolerated, but only purified, tested brands should be used because unpurified products may contain heavy metals or contaminants. Rare adverse effects include a severe allergic reaction (anaphylaxis) and headache; one case of high blood pressure related to mineral imbalances was reported during pregnancy, though a direct link to shilajit wasn't confirmed.
Gokhshur is generally well tolerated short-term in healthy adults, though long-term safety data are limited. Serious but rare complications include liver and kidney injury, seizures, and persistent painful erection; gastrointestinal side effects like nausea, diarrhea, and cramping occur but were no more common than placebo in one study.
Barley is well tolerated as a food and contains gluten; common side effects are bloating, gas, and an unpleasant taste. Allergic reactions including anaphylaxis are possible in sensitive individuals.

The most serious concern is shilajit, which can increase the risk of low blood sugar (hypoglycemia) when taken with antidiabetes drugs like metformin or glibenclamide — this is a Moderate-severity interaction, and animal studies show shilajit can enhance their glucose-lowering effects.

Gokhshur (tribulus) carries three Moderate interactions: it may lower blood pressure further when combined with blood pressure medications, raise the risk of low blood sugar with antidiabetes drugs, and theoretically increase lithium levels in your body, which could require a dose adjustment. The product also contains barley (yavakshar), which in animal studies reduced the absorption of triclabendazole (Egaten), an antiparasitic medication — though this hasn't been confirmed in humans.
We were not able to check several other ingredients: punarnava, pashanbhed, shwet parpati, sahadev, palash pushap, varun ghana, apamarg, and lajjalu mool. Altogether, these interactions span 257 individual medications.

Gokhshur
Antidiabetes Drugs
Taking tribulus with antidiabetes drugs might increase the risk of hypoglycemia.
Clinical research shows that Tribulus can lower blood glucose levels in adults with type 2 diabetes who are taking antidiabetes medications.
Antihypertensive Drugs
Theoretically, taking tribulus with antihypertensive drugs might increase the risk of hypotension.
Animal research shows that tribulus can lower blood pressure by inhibiting angiotensin-converting enzyme (ACE). Tribulus has also demonstrated hypotensive effects in pre-hypertensive adults.
Lithium
Theoretically, tribulus might increase the levels and clinical effects of lithium.
Tribulus is thought to have diuretic properties. Due to these potential diuretic effects, tribulus might reduce excretion and increase levels of lithium. The dose of lithium might need to be decreased.
Shuddha Shilajit
Antidiabetes Drugs
Taking shilajit with antidiabetes drugs might increase the risk of hypoglycemia.
Most human and animal research shows that shilajit can decrease fasting plasma glucose levels. In an animal model, shilajit 100 mg per kg daily enhanced the glucose-lowering ability of both glibenclamide and metformin when given in combination over a 4 week period. Monitor blood glucose levels closely. Dose adjustments might be necessary.
Yavakshar
Triclabendazole (Egaten)
Theoretically, barley might decrease the clinical effects of triclabendazole.
Animal research suggests that a diet supplemented with barley can reduce the bioavailability of triclabendazole when taken concomitantly. This effect has not been shown in humans.
