Shilajit is generally well tolerated, but the safety data advises against it during pregnancy due to limited safety information and contamination risk; avoid it while breastfeeding as well. Unpurified shilajit products may contain heavy metals, so use only purified, tested brands.

Rare adverse effects include allergic reactions (including a case of anaphylaxis with exercise) and headache. One case report linked shilajit use during pregnancy to high blood pressure, though other ingredients or contaminants may have played a role.

Tribulus is often well tolerated short-term in healthy adults, but long-term safety data are limited. Avoid it during pregnancy because it may affect hormones, and avoid it while breastfeeding.

Gastrointestinal side effects — abdominal pain, nausea, vomiting, diarrhea, constipation — can occur, though rates were similar to placebo in one study. Rare serious effects include liver and kidney injury, seizures, and prolonged painful erection.

Barley is safe as a normal food but contains gluten. As a concentrated supplement, pregnancy and breastfeeding safety are not well studied.

Common side effects are bloating, gas, and an unpleasant taste. Allergic reactions (including anaphylaxis in sensitive people) and contact dermatitis have been reported; inhalation of barley flour may cause asthma-like symptoms.

Antidiabetes Drugs

Taking tribulus with antidiabetes drugs might increase the risk of hypoglycemia.
Clinical research shows that Tribulus can lower blood glucose levels in adults with type 2 diabetes who are taking antidiabetes medications.

Theoretically, taking tribulus with antihypertensive drugs might increase the risk of hypotension.
Animal research shows that tribulus can lower blood pressure by inhibiting angiotensin-converting enzyme (ACE). Tribulus has also demonstrated hypotensive effects in pre-hypertensive adults.

Theoretically, tribulus might increase the levels and clinical effects of lithium.
Tribulus is thought to have diuretic properties. Due to these potential diuretic effects, tribulus might reduce excretion and increase levels of lithium. The dose of lithium might need to be decreased.

Taking shilajit with antidiabetes drugs might increase the risk of hypoglycemia.
Most human and animal research shows that shilajit can decrease fasting plasma glucose levels. In an animal model, shilajit 100 mg per kg daily enhanced the glucose-lowering ability of both glibenclamide and metformin when given in combination over a 4 week period. Monitor blood glucose levels closely. Dose adjustments might be necessary.

Theoretically, barley might decrease the clinical effects of triclabendazole.
