The evidence for elderberry isn't established for most of the conditions it's marketed for. Our data shows insufficient reliable evidence to rate it for allergic rhinitis (hay fever), cardiovascular disease, chronic fatigue syndrome, chronic obstructive pulmonary disease, cognitive function, and respiratory tract infections.
That doesn't mean it doesn't work — it means the research so far hasn't been robust enough to say.

Properly cooked elderberry products are generally well tolerated. The real concern is raw or unripe berries, leaves, bark, and seeds — these contain compounds that can cause nausea, vomiting, severe diarrhea, weakness, dizziness, numbness, and even stupor.
Cooking eliminates the toxin, so a commercial extract like this should be safe from that angle. There's also one case report of a woman with an underlying autoimmune condition who developed autoimmune hepatitis after taking elderberry for years; her liver function recovered after she stopped and started steroids.
Because there isn't enough safety data, the facts advise against elderberry during pregnancy and while breastfeeding. If either applies to you, talk with your doctor or pharmacist for personalized guidance.
