Synerpan-5 has four active ingredients: bromelain, protease, lipase, and raw pancreas concentrate. All four are digestive enzymes — they're designed to help break down proteins, fats, and other food components in your digestive tract.
The product also contains gelatin, cellulose, and vegetable stearate as inactive ingredients (the capsule and binders).

Not established
The evidence we hold on these ingredients doesn't show that they're proven to work for any specific condition. Bromelain has been studied for allergic rhinitis (hay fever), burns, cancer, and several other uses, but the evidence in all those cases is insufficient to say one way or the other.
Lipase has similarly uncertain evidence for inflammatory bowel disease and dyspepsia. There aren't effectiveness ratings on file for protease or raw pancreas concentrate.
How safe is it?
Well-documented data
Bromelain is generally well tolerated in the short term, but we don't have much data on long-term use. The most common side effects from bromelain taken by mouth are diarrhea, flatulence, stomach upset, and headache.
Protease may cause digestive upset or allergic reactions in some people. Lipase has no reported adverse effects in adults, though serious gastrointestinal effects (like necrotizing enterocolitis) have been seen in premature infants given a recombinant form — that's not relevant to this product, but it's worth noting.
Allergic reactions to bromelain can happen, especially if you inhale it; symptoms can include throat itching, difficulty swallowing, eye irritation, and in worst cases, asthma. If you're pregnant, the safety data advises against bromelain; protease and lipase don't have enough safety data, so talk with your doctor or pharmacist before using them.
Bromelain should also be avoided while breastfeeding. For protease and lipase, safety while nursing hasn't been well studied — check with your healthcare provider first.

The most serious concern is with blood thinners and antiplatelet drugs (anticoagulants and antiplatelet agents) — bromelain may have antiplatelet effects and increase your bleeding risk if you're already on one of these medications. There's even a case report of minor bruising in someone taking bromelain with naproxen.

Bromelain also has a minor interaction with tetracycline antibiotics. Lab work suggests bromelain might raise tetracycline levels, but a human study showed no change in blood levels when the two were taken together, so the real-world significance isn't clear.

Bromelain
Anticoagulant/Antiplatelet Drugs
Bromelain may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
There is one case report of a patient experiencing minor bruising while taking bromelain with naproxen. Bromelain is thought to have antiplatelet activity. Whether this interaction is of concern with topical bromelain is unclear. Interference with coagulation of burn wounds has been reported in a patient receiving bromelain-based enzymatic debridement. However, observational research has found that topical bromelain debridement is not associated with increases or decreases in laboratory markers of coagulation when compared with surgical debridement.
Tetracycline Antibiotics
Theoretically, bromelain might increase levels of tetracycline antibiotics.
Laboratory research suggests that bromelain might increase the absorption of tetracycline antibiotics. However, a study in healthy adults reported no difference in tetracycline plasma levels when a 500 mg dose was taken with or without bromelain 80 mg.
