Total Enzymes contains nine active ingredients: a proprietary blend of digestive enzymes (lactase, lipase, protease, cellulase, amylase, and invertase), plus chromium, beet, and malt diastase. The enzymes are meant to help break down lactose, fats, and carbohydrates in your digestive tract.
Chromium is a mineral involved in blood sugar control, and beet is included for its nutrient profile. The product also contains two inactive ingredients — microcrystalline cellulose (a binder) and magnesium stearate (a flow agent).
Does it work?
Strong evidence
The evidence for this product's ingredients is mixed. Lactase is effective for lactose intolerance — if you can't digest milk sugar, lactase supplements work.
For the other uses listed (colic and prematurity in infants), the evidence isn't strong enough to rate. Lipase's effectiveness for digestive issues like indigestion (dyspepsia) or inflammatory bowel disease is not established in the data we hold.
Protease (the protein-digesting enzyme) has no effectiveness ratings in our records. Chromium is likely effective for chromium deficiency, possibly effective for diabetes, but possibly ineffective for prediabetes and other conditions we checked.
Beet is possibly effective for athletic performance and exercise-related muscle soreness, but possibly ineffective for lung disease (COPD). Most of the enzyme ingredients lack solid evidence for their specific uses in this supplement.
How safe is it?
Well-documented data
Lactase is generally well tolerated and likely safe in pregnancy and while breastfeeding, though it's wise to check with your doctor before regular use. Lipase is generally well tolerated in supplement form but should only be used under medical guidance if you have serious digestive conditions; safety data in pregnancy and breastfeeding is limited, so ask your doctor first.
Protease is also generally well tolerated but may occasionally cause digestive upset or allergic reactions; it's best avoided in pregnancy unless prescribed, and use without medical advice is not recommended while breastfeeding. Chromium is usually well tolerated at typical doses, though gastrointestinal irritation, headaches, insomnia, and mood changes can occur; high or long-term doses may carry risks, and extra chromium in pregnancy and breastfeeding should be avoided unless your doctor recommends it.
Beet is widely eaten as food and generally well tolerated, though concentrated beet supplements have less safety data and may cause red or black stools (harmless), and rarely, kidney damage if taken in very large amounts. Several ingredients we could not check — cellulase, amylase, invertase, and malt diastase — have no interaction or safety data on file.

Beet
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, beet might increase the levels of CYP3A4 substrates.
In vitro research suggests that betanin, the major pigment in beet, competitively inhibits CYP3A4 in a dose-dependent manner similarly to strong CYP3A4 inhibitor ketoconazole.
Antihypertensive Drugs
Beet and beetroot contain nitrates, which can cause vasodilation, potentially leading to lower blood pressure. However, a study published in the European Journal of Clinical Nutrition using concentrated beetroot juice found no significant impact on blood pressure or heart rate in different age groups. Other small clinical studies suggest that while beet consumption might transiently lower blood pressure due to vessel dilation, there's no consistent evidence of a lasting effect. Overall, the theoretical risk of reduced blood pressure due to beet's nitrate content exists, but studies generally indicate a low and temporary impact rather than a sustained decrease.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, beet might decrease the levels and clinical effects of CYP1A2 substrates.
In vitro research suggests that beet induces CYP1A2 enzymes.
Chromium
Antidiabetes Drugs
Theoretically, chromium may have additive effects with antidiabetic agents and increase the risk of hypoglycemia.
Some research shows that taking chromium might lower blood glucose levels, especially in patients with poorly controlled type 2 diabetes.
Insulin
Theoretically, concomitant use of chromium and insulin might increase the risk of hypoglycemia.
In clinical research, chromium has been shown to increase insulin sensitivity,
Levothyroxine (Synthroid, Others)
Chromium might bind levothyroxine in the intestinal tract and decrease levothyroxine absorption.
Clinical research in healthy volunteers shows that taking chromium picolinate 1000 mcg with levothyroxine 1 mg decreases serum levels of levothyroxine by 17% when compared to taking levothyroxine alone. Advise patients to take levothyroxine at least 30 minutes before or 3-4 hours after taking chromium.
Aspirin
Theoretically, aspirin might increase chromium absorption.
Animal research suggests that aspirin may increase chromium absorption and chromium levels in the blood.
Nonsteroidal Anti-Inflammatory Drugs (Nsaids)
NSAIDs might increase chromium levels in the body.
Drugs that are prostaglandin inhibitors, such as NSAIDs, seem to increase chromium absorption and retention.
