Tribulus is often well tolerated in the short term in healthy adults, but long-term safety data and product quality information are limited. Serious side effects are rare but have been reported, including liver and kidney injury, seizures, and in one case, a painful erection that impaired sexual function.

More commonly, it can cause stomach upset—abdominal pain, cramping, nausea, vomiting, diarrhea, or constipation—though in one study these complaints occurred at similar rates in people taking tribulus and those on placebo. Do not take tribulus during pregnancy because it may affect hormones and safety is not established.

Avoid it while breastfeeding due to lack of reliable safety information.

Taking tribulus with antidiabetes drugs might increase the risk of hypoglycemia.
Clinical research shows that Tribulus can lower blood glucose levels in adults with type 2 diabetes who are taking antidiabetes medications.
