Strong evidence
The evidence for most of V-Zymes' ingredients is limited. Lactase is effective for lactose intolerance.
Lactobacillus acidophilus, Lactobacillus sporogenes, and Bifidobacterium bifidum are possibly effective for irritable bowel syndrome (IBS) and antibiotic-associated diarrhea. The effectiveness of bromelain, lipase, cellulase, invertase, and larch arabinogalactan for their intended uses has not been established in the data we hold—most carry insufficient evidence to rate.
How safe is it?
Well-documented data
Most ingredients in V-Zymes are generally well tolerated short-term. The most common side effects from bromelain are diarrhea, flatulence, and stomach upset; rare allergic reactions (including asthma if inhaled) are possible.
Probiotics can occasionally cause bloating, flatulence, and minor stomach discomfort. Larch arabinogalactan may also cause bloating and gas.
Bromelain is not recommended during pregnancy or while breastfeeding due to insufficient safety data. Lactase is likely safe in pregnancy and is safe while breastfeeding.
For the other ingredients, pregnancy and breastfeeding data are either limited or not established—talk with your doctor or pharmacist if you're pregnant or nursing.

The three probiotic ingredients—Lactobacillus acidophilus, Lactobacillus sporogenes, and Bifidobacterium bifidum—all carry a Moderate interaction with antibiotics. Because these probiotics contain live organisms, taking them at the same time as antibiotics may reduce the probiotics' effectiveness; you'll want to space them apart.
Bromelain also has a Minor interaction with tetracycline antibiotics, where it may theoretically increase their levels in your bloodstream, though study results have been mixed. Larch arabinogalactan carries a Moderate interaction with immunosuppressants (drugs that quiet your immune system), as it may interfere with how those medications work.

Probiotic Blend
Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
Bromelain
Anticoagulant/Antiplatelet Drugs
Bromelain may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
There is one case report of a patient experiencing minor bruising while taking bromelain with naproxen. Bromelain is thought to have antiplatelet activity. Whether this interaction is of concern with topical bromelain is unclear. Interference with coagulation of burn wounds has been reported in a patient receiving bromelain-based enzymatic debridement. However, observational research has found that topical bromelain debridement is not associated with increases or decreases in laboratory markers of coagulation when compared with surgical debridement.
Tetracycline Antibiotics
Theoretically, bromelain might increase levels of tetracycline antibiotics.
Laboratory research suggests that bromelain might increase the absorption of tetracycline antibiotics. However, a study in healthy adults reported no difference in tetracycline plasma levels when a 500 mg dose was taken with or without bromelain 80 mg.
FiberAid Larch Arabinogalactans
Immunosuppressants
Theoretically, larch arabinogalactan might interfere with immunosuppression therapy due to immunostimulant effects. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), corticosteroids (glucocorticoids), and other drugs.
