Does it work?
Strong evidence
Vitamin B12 is effective for treating B12 deficiency and Imerslund-Grasbeck disease (a rare genetic absorption disorder). It's likely effective for cyanide poisoning.
There's possibly effective evidence for canker sores and postherpetic neuralgia (nerve pain after shingles), though the data here is less robust than for deficiency.
How safe is it?
Well-documented data
B12 is generally very well tolerated. Your body removes excess B12 in urine, so there's no established upper safe limit.
Allergic reactions are rare but can happen — rash, itching, and hives have been reported, particularly in people with a cobalt allergy. When B12 corrects severe anemia, a very rare complication of dangerously low potassium (hypokalemia) has been reported, though this is uncommon.
B12 is safe during pregnancy and breastfeeding at normal recommended amounts; follow your provider's guidance on dosing.
