Vitamin B12 and Metformin Hydrochloride

Here is the interaction we found between these two, with the evidence behind it.

Minor

Metformin (Glucophage)

Metformin, a common medication used to manage type 2 diabetes, has been associated with lower vitamin B12 levels in some individuals. Prolonged use of metformin can interfere with the absorption of B12 in the digestive system, potentially leading to a deficiency in this essential vitamin.
Severity Minor Likelihood Possible Evidence A

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's a flip on what people usually expect from an interaction. With metformin (including the Riomet ER form), the medication itself can slowly lower your vitamin B12 levels over time. It does this by interfering with how your gut absorbs B12, and the longer you've been on metformin, the more this can add up. So in this case, B12 isn't changing your metformin, it's the other way around.

Taking a B12 supplement won't hurt your blood sugar control or make metformin work differently, and many people on long-term metformin actually benefit from it. If you've noticed things like tingling in your hands or feet, unusual tiredness, or weakness, mention it to your pharmacist or doctor. A simple blood test can check your B12, and they can advise whether a supplement makes sense for you.

From our pharmacist’s overview on the Metformin Hydrochloride page

“Anyone taking metformin hydrochloride (Riomet ER) should treat this list with genuine care, because two of its 257 entries are rated major. Siberian cocklebur has caused severe hypoglycemia in people and should not be combined with metformin at all, and activated charcoal can adsorb the drug in the gut so less reaches your blood.”

About Vitamin B12

What is Vitamin B12?

Vitamin B12
  • Treating or preventing B12 deficiency
  • Pernicious anemia
  • Low energy and fatigue
  • Nerve health and tingling/numbness
  • Support for vegetarians and vegans

Vitamin B12 (cobalamin) is an essential nutrient your body needs to make red blood cells, keep nerves healthy, and support DNA. Supplements are very helpful for people who are deficient — such as vegans, older adults, and those with absorption problems — but taking extra B12 when you are not deficient is unlikely to boost energy or health. Talk to your pharmacist or doctor if you think you may be low.
