Vitamin B12 and Alogliptin, Metformin

Here is the interaction we found between these two, with the evidence behind it.

Minor

Metformin (Glucophage)

Metformin, a common medication used to manage type 2 diabetes, has been associated with lower vitamin B12 levels in some individuals. Prolonged use of metformin can interfere with the absorption of B12 in the digestive system, potentially leading to a deficiency in this essential vitamin.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's something good to know about Kazano: one of its two ingredients is metformin, and over months to years metformin can quietly lower your vitamin B12 levels. It does this by making B12 harder to absorb in your gut. The alogliptin part of Kazano isn't the player here, it's the metformin. This is a well-documented effect, though it doesn't happen to everyone.

The nice news is that taking a B12 supplement works with your Kazano, not against it. It simply helps replace what metformin may deplete, which can guard against tiredness, tingling in the hands or feet, or anemia from low B12. It won't change how well your diabetes is controlled. Ask your pharmacist or doctor about checking your B12 level so you take the right amount.

From our pharmacist’s overview on the Alogliptin, Metformin page

“Kazano combines alogliptin and metformin to lower blood sugar, and nearly everything on this list pushes on that same lever, so I treat these pairings with real caution. Most entries are herbs and supplements that lower glucose on their own, and stacked on the drug they raise the risk of hypoglycemia, a sugar drop steep enough to cause…”

About Vitamin B12

What is Vitamin B12?

Vitamin B12
  • Treating or preventing B12 deficiency
  • Pernicious anemia
  • Low energy and fatigue
  • Nerve health and tingling/numbness
  • Support for vegetarians and vegans

Vitamin B12 (cobalamin) is an essential nutrient your body needs to make red blood cells, keep nerves healthy, and support DNA. Supplements are very helpful for people who are deficient — such as vegans, older adults, and those with absorption problems — but taking extra B12 when you are not deficient is unlikely to boost energy or health. Talk to your pharmacist or doctor if you think you may be low.
