Not established
Vitamin B12 is effective for treating B12 deficiency and Imerslund-Grasbeck disease (a rare genetic condition affecting B12 absorption). It's likely effective for cyanide poisoning.
The evidence also suggests it may help with canker sores and postherpetic neuralgia (nerve pain after shingles), though the data for those uses is less complete.

Well-documented data
B12 is generally very well tolerated — your body excretes excess amounts in the urine, so there's no established upper limit. Allergic reactions are rare but possible; they may include rash, itching, or hives, and theoretically could relate to the cobalt in the B12 molecule itself.
Very rarely, correction of severe anemia with B12 has been linked to serious low potassium (hypokalemia), though this is extremely uncommon. B12 is safe during pregnancy and breastfeeding at recommended amounts — in fact, it's needed during both.
