Nopal cactus is generally well tolerated when taken by mouth. The most common side effects are mild: abdominal fullness, indigestion, diarrhea, and nausea.

Rare side effects include headache, insomnia, and dizziness—a clinical study of 41 people saw two with headache, one with dizziness, and one with insomnia. Cases of obstruction in the colon and rectal injury have been reported, but only in people who ate a very large number of prickly pear fruits; these are extremely rare.

Because the dried cactus swells when it absorbs water, it can increase stool volume and frequency. There isn't enough safety data to know whether nopal cactus is safe in supplement amounts during pregnancy or while breastfeeding.

Combining prickly pear cactus with antidiabetes drugs might increase the risk of hypoglycemia.
Case reports show that combining prickly pear cactus with antidiabetes drugs such as chlorpropamide, glyburide, glipizide, and metformin can increase the risk of hypoglycemia in patients with type 2 diabetes. Advise patients to monitor glucose levels closely. Dose adjustments may be necessary.
