Human safety data for gardenia are limited, and we don't know much about what happens with large or long-term use. Topically (on the skin), it can cause allergic contact dermatitis in sensitive people.

There's also a single case report of blue-gray skin discoloration linked to taking a multi-ingredient product containing gardenia extract over 7 years, thought to be from a gardenia component called genipin enhancing melanin deposits — the patient improved after stopping. During pregnancy, there isn't enough safety information, so it's best to avoid it.
