Bleomycin must be given under the supervision of a physician experienced in cancer chemotherapy. The most serious risk is lung toxicity — specifically, a type of lung inflammation called pneumonitis that can progress to pulmonary fibrosis (permanent scarring of the lungs). This risk increases with higher total doses (over 400 units) and in elderly patients, but it has also occurred in younger patients at lower doses. In addition, about 1% of lymphoma patients have experienced a severe reaction — including low blood pressure, mental confusion, fever, chills, and wheezing — which requires immediate medical attention. Bleomycin's main action is to interfere with a cancer cell's DNA. It causes breaks in the DNA strands — both single and double-strand breaks — which the cell cannot easily repair.
