Overview

Shiitake is an edible mushroom that has been used for food and in traditional medicine in East Asia for centuries. It grows naturally on the wood of certain hardwood trees and is now widely cultivated around the world.

People eat shiitake fresh or dried as part of meals, and it is also sold as a dietary supplement in capsules, powders, and liquid extracts. Some specialized extracts taken from shiitake, such as lentinan and AHCC, are studied more often in medical research, usually for immune support.

How it works

Shiitake contains compounds that researchers believe may affect the body in several ways. The best known is lentinan, a type of beta-glucan (a complex sugar) thought to interact with the immune system. Shiitake also contains a compound called eritadenine that has been studied for possible effects on cholesterol.

The mushroom also provides antioxidants and other natural substances that may help protect cells. It is important to know that much of this information comes from laboratory and animal research, and these effects have not been clearly proven in people taking common supplement forms.

Safety & precautions

Eating shiitake as a normal food is considered safe for most people. Concentrated supplements are less studied, so use them with care and ideally under the guidance of a healthcare professional.

Raw or undercooked shiitake can cause a skin reaction in some people known as "shiitake dermatitis," an itchy rash. Cooking the mushrooms thoroughly usually prevents this. People with mushroom allergies should avoid shiitake.

Pregnancy and breastfeeding: Eating shiitake as food is likely fine, but there is not enough safety information on concentrated shiitake supplements during these times. Orally, shiitake mushrooms can cause shiitake dermatitis, a skin eruption that resembles whiplash marks, usually found on the trunk and limbs. This dermatitis is thought to be a toxic response to lentinan or other compounds found normally in uncooked or inadequately cooked shiitake mushroom. The rash can be made worse by scratching. Symptom onset is usually within hours to days and can persist for 3-4 weeks before resolving on its own. There is some evidence that treatment with steroids alone or with antihistamines might reduce the duration of the rash by a small amount in some people. The dermatitis may include small purple spots from broken capillaries, skin plaques, burning, blanching, and pustules. Rarely the rash may look like measles rather than whiplash. Histologically, there may be evidence of dermal and epidermal edema, lymphocyte infiltration, and skin thickening. Other symptoms associated with the dermatitis include fever, aching, malaise, eosinophilia, diarrhea, prickling in the hands, trouble swallowing, conjunctivitis, and pustules with small ulcers in the mouth. It is likely that the dermatitis and other symptoms are due to a delayed type hypersensitivity reaction. Cooking shiitake mushroom generally prevents shiitake dermatitis, although some cases have occurred in people who have consumed cooked sources. It appears that to inactivate lentinan, cooking temperatures of at least 130°C are needed.

Less common is a photosensitivity reaction associated with oral ingestion, which involves rash and pruritus after sun exposure.

Orally, the shiitake mushroom extract AHCC has been reported to cause mild itching.

Orally, shiitake mushrooms can cause abdominal discomfort, including bloating, nausea, pain, vomiting, and diarrhea. Gastrointestinal symptoms, such as diarrhea, problems swallowing, or mouth ulcers have been associated with shiitake dermatitis. Consumption of large pieces of shiitake mushroom with inadequate chewing can cause abdominal obstruction that has resulted in death in one case and surgical intervention in two others. In another case, parenteral nutrition was used exclusively until the shiitake mushroom pieces were passed.

Topically, an oral rinse containing shiitake mushroom extract has been associated with teeth sensitivity, teeth staining, and burning in the mouth.

Dosing

There is no official standardized dose for shiitake supplements, and products vary widely in strength and form. Some research used specialized extracts (like lentinan given by injection in a medical setting), which is very different from over-the-counter capsules.
