How we grade severity & evidence

Severity levels

  • Contraindicated: These should generally not be used together.
  • Major: Potentially serious — often needs a change or close monitoring.
  • Moderate: Can be significant — usually manageable with monitoring.
  • Minor: Usually limited clinical impact.

Evidence grades

  • Established: Well documented — supported by controlled studies or strong clinical data.
  • Probable: Good supporting evidence, though not definitively proven.
  • Suspected: Some evidence suggests this interaction, but it is not well established.
  • Possible: Limited or conflicting evidence; the interaction may occur.
  • Theoretical: Predicted from the drugs' pharmacology; not yet confirmed in people.

Ratings come from the documented interaction literature and are reviewed by a pharmacist. They describe the documented risk of the combination, not what will necessarily happen to you — your dose, timing, and health picture all matter.

Of 284 documented Dabigatran interactions, 29 are rated moderate — including this one.

a) In a drug interaction study, concurrent administration of dabigatran etexilate 150 mg with darunavir/ritonavir 800/100 mg single dose resulted in increased exposure of dabigatran etexilate. The least-mean square ratio of AUC and Cmax of dabigatran etexilate with or without darunavir were 1.72 (90% CI, 1.33 to 2.23) and 1.64 (90% CI, 1.21 to 2.23) respectively. In another drug interaction study, concurrent administration of dabigatran etexilate 150 mg with darunavir/ritonavir 800/100 mg every day for 14 days resulted in the least-mean square ratio of AUC and Cmax of dabigatran etexilate with or without darunavir were 1.18 (90% CI, 0.9 to 1.53) and 1.22 (90% CI, 0.89 to 1.67) respectively 1.
