Coadministration of digoxin and lansoprazole may increase digoxin exposure. Measure digoxin exposure before initiating concomitant drugs. Continue monitoring and reduce digoxin dose as necessary. Also, monitor potential signs and symptoms of clinical toxicity is necessary when initiating, adjusting, or discontinuing lansoprazole23. Lansoprazole produces extensive and long-lasting inhibition of gastric acid secretion. For some drugs, including digoxin, absorption from the gastrointestinal tract is enhanced at higher pH. When digoxin is coadministered with lansoprazole, reduced acidity may increase digoxin absorption, thus increasing its bioavailability. Consider monitoring magnesium levels prior to initiation of lansoprazole (PPI) and periodically 1.
