Concomitant use of lurasidone with a strong CYP3A4 inhibitor (posaconazole) is contraindicated as coadministration may increase lurasidone exposure1. Concomitant use has decreased clearance resulting in a 6.2-fold increase in lurasidone exposure. Consider limiting the dosage of lurasidone or implementing a washout period of 2 weeks in normal weight patients and 3 weeks in obese patients after discontinuation of posaconazole 2.

a) Coadministration of lurasidone and posaconazole resulted in a 6.2-fold increase in lurasidone AUC in healthy normal weight volunteers (n=11) and a 4.9-fold increase in healthy obese volunteers (N=13; mean BMI 49.3 kg/m(2)). Washout of posaconazole was slow in normal weight (mean half-life 31 hours), and was further prolonged in obese volunteers (53 hours). AUC remained significantly elevated in both normal weight (factor of 2.1) and obese volunteers (factor of 3.4) even at 2 weeks after stopping posaconazole. Adverse events occurred in 5 patients including gastrointestinal events (n=2), drug mouth (n=1), somnolence (n=1), and headache (n=1); all resolving without treatment 2.
