Why this applies to Ifosfamide

Ifosfamide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group African Wild Potato interacts with.

In vitro evidence shows that African wild potato extract can inhibit cytochrome P450 3A4 (CYP3A4) enzyme activity by up to 86%. Theoretically, African wild potato might increase levels of drugs metabolized by CYP3A4. However, in human research, African wild potato does not seem to affect the pharmacokinetics of Efavirenz, a CYP3A4 substrate. Until more is known, use African wild potato cautiously or avoid in patients taking drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's a wrinkle that matters with ifosfamide: it's actually a prodrug. Your liver's CYP3A4 enzyme has to switch it on into its active, cancer-fighting form. African Wild Potato, at least in test-tube studies, can block that same enzyme quite strongly. So in theory, the supplement could slow the activation of ifosfamide and leave you with less of the active drug doing its job against the cancer.

I want to be straight with you, though: this concern comes only from lab data, not from studies in people, and in one human study the potato didn't change a similar drug's levels. Still, ifosfamide is serious chemotherapy and not something to gamble with. Please don't add this supplement without first clearing it with your oncologist or pharmacist.
