Why this applies to Dextromethorphan

Dextromethorphan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group African Wild Potato interacts with.

In vitro evidence shows that African wild potato extract can inhibit cytochrome P450 3A4 (CYP3A4) enzyme activity by up to 86%. Theoretically, African wild potato might increase levels of drugs metabolized by CYP3A4. However, in human research, African wild potato does not seem to affect the pharmacokinetics of Efavirenz, a CYP3A4 substrate. Until more is known, use African wild potato cautiously or avoid in patients taking drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist with this pairing. In a lab dish, African wild potato slowed down an enzyme called CYP3A4, which is one of the pathways your body uses to break down dextromethorphan (a partner enzyme, CYP2D6, is the main one). If that breakdown slows, a bit more dextromethorphan could linger, which in theory might bump up its usual side effects like drowsiness, dizziness, or an upset stomach.

I want to be honest about the strength here: this is only test-tube evidence, and in actual people the potato didn't change levels of another similar drug. So the real-world risk looks low. Still, if you take both and notice feeling extra groggy, check in with your pharmacist or doctor before changing anything.

Dextromethorphan temporarily relieves cough due to minor throat and bronchial irritation, such as the kind that comes with the common cold or breathing in irritants. It also helps reduce the urge to cough so you — or your child — can get to sleep.

Some products, including CVS Long Acting Cough and Premier Value Tussin Cough oral liquids, are also indicated to help loosen phlegm (mucus) and thin bronchial secretions to help drain the airways.
