Why this applies to Irinotecan

Irinotecan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group African Wild Potato interacts with.

In vitro evidence shows that African wild potato extract can inhibit cytochrome P450 3A4 (CYP3A4) enzyme activity by up to 86%. Theoretically, African wild potato might increase levels of drugs metabolized by CYP3A4. However, in human research, African wild potato does not seem to affect the pharmacokinetics of Efavirenz, a CYP3A4 substrate. Until more is known, use African wild potato cautiously or avoid in patients taking drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the situation with irinotecan (Camptosar). Your liver uses an enzyme called CYP3A4 to help process this chemotherapy drug. In test-tube studies, African Wild Potato strongly blocked that same enzyme. The worry is that, in theory, slowing down CYP3A4 could let more irinotecan build up in your body, which might mean harsher side effects like severe diarrhea, low blood counts, nausea, and a higher risk of infection.

I want to be honest about the evidence here. This concern comes only from lab work, not from people, and one human study with a similar drug showed no real effect. Still, because irinotecan is potent chemo with a narrow safety margin, I'd play it safe. Please don't add this supplement without first clearing it with your oncology team or pharmacist.
