Netupitant, Palonosetron (Oral) falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group American Elder interacts with.
In vitro research shows that American elder inhibits CYP3A4 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Akynzeo (netupitant/palonosetron). The netupitant part is handled by a liver enzyme called CYP3A4. In test-tube studies, American elder appears to slow that same enzyme down. If that holds true in people, your body might clear netupitant a little more slowly, letting levels drift higher than usual. With this medicine that could mean more of its typical side effects, like headache, constipation, tiredness, or feeling weak.
I want to be honest about the strength of this, though. It's theoretical, based only on lab work, not real patient studies, so I wouldn't lose sleep over it. Akynzeo is also usually a short, one-time dose with chemo. Still, since you're going through treatment, just mention any elderberry product to your oncology team or pharmacist first.

American elder is a North American plant whose cooked, ripe berries and flowers are used mainly for colds, flu, and general immune support. The science behind these uses is limited and most studies focus on the closely related European black elder (Sambucus nigra), so benefits are uncertain. Only properly prepared berries and flowers should be used, since raw berries, leaves, bark, and seeds can cause poisoning.

What Is Netupitant and Palonosetron Used For?
Brand name: Akynzeo
Netupitant and palonosetron helps prevent nausea and vomiting from cancer chemotherapy in adults. It is used together with dexamethasone, a steroid medicine. It covers both the first day (acute phase) and the days after (delayed phase).
Akynzeo capsules are used for initial and repeat courses of chemotherapy, including highly emetogenic (strongly nausea-causing) chemotherapy. Akynzeo for injection and Akynzeo injection are used for highly emetogenic chemotherapy only. They have not been studied with anthracycline plus cyclophosphamide chemotherapy.
