Pentoxifylline falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Arnica interacts with.

Theoretically, arnica might have additive effects with anticoagulant and antiplatelet drugs. Homeopathic arnica preparations are unlikely to have this interaction.
In vitro evidence shows that sesquiterpene lactones in arnica flowers can decrease platelet aggregation. However, this effect has not been reported in humans.

Here's the thinking behind pairing arnica with pentoxifylline (Trental). Pentoxifylline already works by thinning your blood a bit so it flows more easily through tiny vessels. Lab studies suggest arnica's natural compounds may also make platelets a little less sticky. Stack the two together and, in theory, you could lean toward easier bruising, nosebleeds, or bleeding that takes a bit longer to stop.

The good news is this is mostly a theoretical concern. It's only been seen in test-tube studies, not in real people, and gentle homeopathic or skin-only arnica products are very unlikely to cause a problem. Still, if you plan to take arnica by mouth, check with your pharmacist or doctor first.

What Is Pentoxifylline Used For?

Pentoxifylline Extended-Release Tablets are used to treat intermittent claudication — leg pain, cramping, or fatigue that happens during walking because narrowed arteries aren't delivering enough blood to the muscles. The underlying cause is chronic occlusive arterial disease, a condition where the arteries in the limbs become blocked or narrowed over time.

Pentoxifylline can help improve walking distance and reduce symptoms, but it is not a substitute for procedures that physically open or bypass blocked arteries when those treatments are needed.
