Belumosudil Mesylate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Astaxanthin interacts with.

Theoretically, astaxanthin may decrease levels of drugs metabolized by CYP3A4.
In vitro research shows that astaxanthin induces CYP3A4 enzyme activity in human hepatocytes. This effect has not been reported in humans.

Here's the gist for Rezurock (belumosudil). Your body clears this medicine using a liver enzyme called CYP3A4. In lab dish studies, astaxanthin seemed to speed that enzyme up. If the same thing happened in people, your system could break belumosudil down a little faster, leaving less of it in your blood and possibly making it work less well against your graft versus host disease.

The important part: this is only a theoretical concern from test-tube research, and it has never actually been seen in humans. So I wouldn't lose sleep over it. Still, because belumosudil is doing a serious job for you, please run any astaxanthin supplement past your transplant team or pharmacist before you start it.

Rezurock (belumosudil) is used to treat chronic graft-versus-host disease (chronic GVHD) — a serious immune condition that can occur after a stem cell or bone marrow transplant, where the donor's immune cells attack the recipient's body. It is specifically for patients aged 12 and older whose chronic GVHD has not responded to at least two prior systemic (whole-body) treatments.
