Astaxanthin and Omeprazole

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Omeprazole

Omeprazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Astaxanthin interacts with.

Theoretically, astaxanthin may decrease levels of drugs metabolized by CYP3A4.
In vitro research shows that astaxanthin induces CYP3A4 enzyme activity in human hepatocytes. This effect has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version: omeprazole gets cleared from your body partly by a liver enzyme called CYP3A4. In lab dish studies (not in actual people), astaxanthin seemed to speed that enzyme up. If that happened in your body, your omeprazole could be broken down a little faster, which in theory might leave less of it working. For a stomach acid medicine, that could show up as your heartburn or reflux not being controlled as well as usual.

The reassuring part: this is only a test-tube finding and has never been seen in real patients, so it may not matter at all. Omeprazole is also pretty forgiving. If you take both and notice your reflux creeping back, just mention it to your pharmacist or doctor before making any changes.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

These ratings are for information only. Never start, stop, or otherwise change your therapy before speaking with your provider.

From our pharmacist’s overview on the Omeprazole page

“Omeprazole (Prilosec, Zegerid, Losec) is an acid reducer for heartburn, reflux, and ulcers, and because it sits on pharmacy shelves next to the vitamins, people tend to assume it mixes safely with anything. This list argues otherwise, and it warrants genuine care.”

About Astaxanthin

What is Astaxanthin?

Astaxanthin
  • Antioxidant support
  • Skin health and aging
  • Eye health
  • Exercise recovery
  • Heart and inflammation support

Astaxanthin is a reddish carotenoid pigment with strong antioxidant activity in the lab, and it is widely promoted for skin, eye, heart, and exercise benefits. Early human studies are promising for a few uses, but the overall evidence is still limited and not strong enough to make firm claims. It appears generally well tolerated at typical supplement doses, but talk with your pharmacist or doctor before starting it.

About Omeprazole

What Is Omeprazole Used For?

Brand names: Prilosec, Zegerid, Losec

Prescription omeprazole delayed-release capsules and Prilosec are used for several acid-related conditions. These include active duodenal ulcer, active benign gastric ulcer, and gastroesophageal reflux disease (GERD). They are also used for erosive esophagitis (damage to the esophagus lining from acid) and pathologic hypersecretory conditions (when the stomach makes too much acid).

With the antibiotics clarithromycin, with or without amoxicillin, omeprazole is used in adults to clear H. pylori infection in people with duodenal ulcer disease. Over-the-counter omeprazole tablets and capsules are for treating frequent heartburn, which is heartburn on 2 or more days a week.
