Ensartinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Astaxanthin interacts with.

Theoretically, astaxanthin may decrease levels of drugs metabolized by CYP3A4.
In vitro research shows that astaxanthin induces CYP3A4 enzyme activity in human hepatocytes. This effect has not been reported in humans.

Ensartinib (Ensacove) is a targeted cancer medicine used for certain ALK-positive lung cancers, and your liver clears it using an enzyme called CYP3A4. Here's the theoretical concern: lab research suggests astaxanthin might speed up that enzyme. If that happened in real life, your body could break down ensartinib a little faster, which might lower its levels and, in theory, make it work less well against the cancer. Ensartinib is not a prodrug, so faster clearance means weaker effect, not stronger.

I want to be honest about how thin this evidence is. This effect has only been seen in test-tube studies, never confirmed in people. Still, with a cancer therapy you don't want to gamble on lost effectiveness, so please check with your oncologist or pharmacist before adding astaxanthin.
