Nirmatrelvir, Ritonavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Astaxanthin interacts with.

Theoretically, astaxanthin may decrease levels of drugs metabolized by CYP3A4.
In vitro research shows that astaxanthin induces CYP3A4 enzyme activity in human hepatocytes. This effect has not been reported in humans.

Here's the gist with Paxlovid (nirmatrelvir plus ritonavir). Part of how this combo works is that ritonavir purposely blocks an enzyme called CYP3A4 so the antiviral nirmatrelvir stays at higher, effective levels in your body. In lab dish studies, astaxanthin seemed to do the opposite and speed that enzyme up. If that happened in real life, it could in theory nudge nirmatrelvir levels down a bit, which might make a short COVID treatment course work less reliably.

That said, this is only seen in test-tube cells, never confirmed in people, so I wouldn't lose sleep over it. Paxlovid is just a 5-day course anyway. Still, run it past your pharmacist or doctor before mixing the two.
